Before I schedule a tour, I ask parents to read my contract carefully.
After this step, if you are still interested in a spot or my waiting list, I will send a questionnaire to be completed. After the questionnaire has been reviewed, I will schedule a tour.
This is done to better determine if my program will be a good fit for everyone involved.

My contract

The Parent is required to ensure that all forms are completed in their entirety and returned to the Provider before the child(ren) enters the program. All forms must be renewed and completed on an annual basis in an effort to ensure that the Provider has the most accurate and up to date information on each child/family. Children entering the program will be given a 2 week trial basis, during which time either party may cancel participation without notice. At the end of the trial period, both the Provider and the Parent will meet to discuss the child's comfort and adaptability with our program and setting. An advance payment for the trial period is required for the entire trial period, but there is no obligation to continue in the program after the trial period has ended. Any cancellation during the trial period, will be refunded for the remaining days of the trial period.

HOURS OF OPERATION

The Provider is open from 7:30 am to 4:30 pm, Monday through Friday

STANDARD RATES AND PAYMENT POLICIES

Child Care Fees: Full-Time Infants (0-18 month old ): $ 325 /week, Full-Time Toddlers (2-5 y/o): $275 / week 

Holidays and child absences shall be billed as if care had been provided.

Paid Holidays shall include: New Years Day, Martin Luther King Day, President’s Day, Memorial Day, June Nineteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day

A deposit equal to your expected weekly rate is required.

The deposit will be applied to the last week’s payment or to the termination notice period if proper notice is not given (see Termination Procedures). Day Care fees shall be due and payable on the Friday, or the last business day of each week, in advance of the coming week. In the event that the program fee is not paid by the due date, a late fee of $20 per "Late Day" shall be charged. If the payment (including late charges) is not received within 7 days of original due date, the Provider shall immediately terminate this Day Care Agreement and enrollment of the child(ren). If the child(ren) enrolls in day care on any day other than on a Monday, then payment for the first week is to be prorated to cover Day Care provided for the remainder of the week and is due and payable upon the first day of enrollment.

 

LATE PICK-UP & EARLY DROP-OFF POLICY AND FEES

Early Drop-Off: The Provider will charge an Early Drop-off fee of $1.00 per minute that the Parent arrives prior to the agreed upon drop-off time. This fee is due and payable to the Provider on site at time of drop-off.

Late Pick-Up : The Provider will charge a Late Pick-up fee of $5.00 per minute that the Parent is late from the agreed upon pick-up time for their child(ren). This fee is due and payable to the Provider on site on the day of the late pick up.

ACCEPTED PAYMENT METHODS, TERMS AND POLICY

The Provider accepts the following forms of payment: a) Cash b) Check c) Venmo - Fees will be charged d) Direct deposit The Parent shall be responsible for any and all fees, which may be charged by the bank, with regards to any check returned for non-sufficient funds or any payment declined due to lack of funds or credit available.

 

TERMINATION FOR PERSISTENT LATE OR RETURNED PAYMENTS

Persistent late payments shall be considered grounds for termination of Day Care services. The Parent shall be charged a "Contract Termination Fee" for non-payment, consisting of the initial 2 deposit. The Provider may terminate Day Care Services for those Parents whose accounts are days past due without advance notice. Parents shall be held responsible for any and all legal and/or collection fees (court, attorney and collection agency fees) related to the collections of any outstanding balances.

 

PROVIDER / CHILD(REN) VACATIONS

The Provider shall take 1 PAID weeks vacation per year. The Provider will provide the Parent(s) a minimum of four weeks' notice with regards to vacation plans to afford the Parent(s) the opportunity to find reliable and alternative day care during Provider's vacation. Any additional vacation time shall be free of charges. The Parent shall provide 2 weeks written notice for any planned family vacation. These two weeks will be given at half of their normal weekly rate. There is a maximum discount of 2 weeks per enrollment year. However, if the Parent fails to provide the specified advance written notice, then the Parent shall be charged the regular fees for Day Care during their Vacation.

 

PROVIDER PERSONAL & TRAINING DAYS

In addition, the Provider reserves the right to take personal days and training days, of which the Provider shall give the Parent a minimum seven days advance notice. The Parent is expected to pay for Day Care services on any Provider training days.

 

PROVIDER SICK DAYS

In the unlikely event that the Provider shall fall ill or is faced with an emergency, the Provider will notify you as soon as possible for you to find alternative care. The Parent will not be charged for Provider’s sick days.

ILLNESS POLICY

In the event that your child should show any signs of an oncoming illness, please notify the Provider immediately, as the Provider has the obligation to protect the other children at the Day Care facility from illness whenever possible. If you child displays any symptoms or conditions below, they should be kept home or shall be sent home if the following conditions below becomes apparent: • the illness prevents the child from participating in the program activities or from resting comfortably • the child has any of the following conditions: fever, unusual lethargy, irritability, persistent crying, difficult breathing, or other signs of serious illness; • strep infection, until 24 hours after treatment and the child has been without fever for 24 hours • Fever over 101 F 3 • vomiting two or more times in the previous 24 hours at home or once at the center • rash with a fever or behavior change until the physician has determined that the illness is not a communicable disease • Heavy nasal discharge (unless related to allergy and accompanied by a doctors' note) • Diarrhea • purulent conjunctivitis (defined as pink or red conductive with white or yellow discharge, often with matted eyelids) until examined by a physician and approved for re-admission, with or without treatment • Parasites (e.g. nits, lice, crabs, scabies etc.) • Communicable diseases (e.g. chicken pox, measles, ringworm, scarlet fever etc.). A child may also be sent home when the Provider believes, in their judgment, that the child's condition poses a threat to the health or safety of other children or staff in the program. Parents of children enrolled with the Provider will be notified of any contagious illness or diseases that their child(ren) may have been exposed to. In the event a child becomes ill at the Day Care facility, he or she will be separated from the rest of the children and the Parent will be called. The Parent, or Authorized Representative, MUST pick-up the child within one hour after notification of illness. After one hour, the Parent will be charged any additional costs related to private, isolated care. The child should be symptom free for 24 hours before returning to the Facility. Should a child be or become sick during one of their scheduled days, the Parent is still responsible for Payment of those days to the Provider. Payments are required to reserve and hold the Child’s spot, therefore regardless of the actual attendance of the Child, payment is still required. No prorated day or week and/or makeup time is required from the Provider.

 

TERMINATION PROCEDURES

  • Provider Required Advance Notice

    • The Provider may terminate any child's enrollment upon 1 weeks advance notice to the Parent for any reason. The pre-paid deposit can be applied towards the final weeks of attendance. Should there be any outstanding debts owed by the Parent such that the deposit does not cover the outstanding debt plus the last weeks of child care, the Provider shall apply the prepaid deposit first to the outstanding debt. If Provider's notice of termination occurs in the midst of a longer pre-paid payment period, then a prorated amount will be refunded to the Parent after first deducting any outstanding balances owed. 

  • Provider Immediate Termination\

    • The Provider may terminate a child's enrollment in the Day Care immediately, if any of the following conditions arise:

      • At the Provider's sole discretion, it is decided that the child's behavior or that of the Parent poses a significant threat to the physical or mental health or well-being of any of the children, 4 staff, the program or other persons on the Provider's premises, and the Provider is unable to reasonably eliminate the threat.

      • Any payment owed by the Parent to Provider under this agreement is not paid within seven days after is due.

      • The child is picked up late, more than five times in any thirty (30) day period.

      • If your child can no longer nap or have a quiet time.

If pursuant to any of the reasons set forth above, the Provider terminates the child's enrollment in the midst of a payment period (monthly or weekly); a prorated amount will be refunded to the Parent after first deducting any outstanding balances owed. The Provider will also refund the prepaid deposit for the last week of attendance, after first deducting any outstanding balances that remain due and owing.

  • Parent Termination

    • The Parent may terminate this agreement with at least 3 weeks advanced notice.

 

MEDICAL CONDITIONS / ALLERGIES

The Parent must fill out a form provided by the Provider listing all of the child's allergies and all medical conditions.

DISCIPLINE

The Provider will not make use of any corporal punishment in the discipline of the children. Should discipline be required, the Provider shall use redirection and/or a time out.

DAMAGES

All children enrolled at Day Care are expected to treat all property located at and within the Facility with respect. The Parent(s) agree to pay for any accidental or willful destruction of any property located at the Facility, whether said property is owned by the Provider or any other person, at the current replacement cost, if such damage and/or destruction were caused by the child.

 

MODIFICATION/AMENDMENT

The Provider reserves the right to modify and/or amend this agreement upon 2 weeks' advance written notice of any changes in the basic rates or services; however, provided that any government requirements change, rate and policy changes can be effective immediately and do not require any prior notice to Parent. Changes in the basic rates and services do not require 5 Parent’s consent, but all other changes require Parent’s consent. The Provider has the right to modify and/or amend this agreement to reflect changes in the rules and policies with regard to and including those changes reflected in the Provider handbook.

 

COMMUNICATION BETWEEN PARENT & PROVIDER

The Parent herein acknowledges and agrees to notify the Provider of any changes in employment and ability to pay, any absences as soon as the parent becomes aware that the child shall be unable to attend on a particular day, or any other event which may have an impact on the terms and conditions contained within this Agreement. In addition, the Parent shall notify the Provider of any changes in their child's health and/or illness immediately upon discovery.

 

NAP/REST TIME

All children will be required to nap or rest quietly after lunch each day. Rest time is important to ensure children are ready to learn and play, so it is not an optional part of the program. Older children who may be outgrowing naps will be provided with a quiet activity or tv, while the other children sleep. I will not fight to keep a child awake and not fight to make them go to bed. I do try my best to make accommodations for the child to stay in my program. EEC Child Care regulations require that an extended period of sleep, rest or quiet activities be provided for children in care for longer than four hours. Children who choose not to sleep or awaken early must be offered quiet activities for the remainder of the sleep or quiet activity period [See 606 CMR 7.11(13), 7.06(1)], [102 CMR 7.24(5)]

REQUIREMENTS FROM PARENT

The Parent is responsible to provide the Provider with formula, diapers/pull-ups, wipes, a change of clothes, portable changing pads, and all other necessary supplies, for their child(ren) who are still using diapers. By signing this, you are documenting that we are in agreement about the hours your child will receive care, and the rates you will be paying for that care. You are also stating that you understand my policies regarding late fees, termination, and any other issues documented above.

 COVID-19

If a daycare child tests positive:

 

The child can return to daycare 10 days after the date of testing if all symptoms are gone and the child is fever free without fever-reducing medication for at least 48 hours.

 

If a daycare child is close contact: 

 

The child can return to daycare 10 days after initial exposure, as long as the child has experienced NO symptoms and tested negative after day 5.

 

A child must provide certification of a negative test (antigen or PCR) taken on day 5 or later because children cannot reliably mask

 

If the child experiences symptoms during a 10 day isolation period, the child will need to be tested. If negative, the child can return to daycare after the isolation period and all symptoms are gone.

 

REPORTING SUSPECTED OR CONFIRMED CASES IN HOUSEHOLD IMMEDIATELY IS REQUIRED

 

PAYMENTS (COVID only)

 

Any payment due during this period will be discounted 50% of the weekly tuition.

 

These protective measures have been put in place to enhance the safety and health for all of our families. Your anticipated cooperation is sincerely appreciated.


 

If Provider or someone in Provider’s household tests positive 

 

  • Families of children will be informed immediately

  • The Facility will be closed for a minimum of 5 days or until person tests negative  

  • Payments will be discounted 100%
